How to reduce neck pain at a desk
A stiff, aching neck at the end of a screen day is usually your head paying for where your screen sits. A neck that aches by the afternoon is one of the most common complaints from a desk, and one of the most misdiagnosed. The usual advice is to sit up straight and hold your head correctly, as though the fix were a matter of remembering to. It rarely is. A sore neck at a desk is almost always your head paying, all day, for where your screen has forced it to go — and the useful thing to change is the screen, not your willpower.
OSHA describes the position a neck is comfortable in plainly. In its neutral seated posture the “head is level, forward facing, and balanced,” and “generally it is in-line with the torso.” That word balancedis doing the work. A head balanced over the spine is held up by the skeleton beneath it, and the muscles barely have to work. Tip it forward, even a little, and it stops being balanced and starts being held — and the muscles at the back of the neck are what hold it, hour after hour, until they tell you about it.
So this page walks the setup causes that pull a head out of balance, roughly in the order they matter: the screen height, the laptop, the phone, the screen off to one side, and the hours you spend without moving at all. The most common cause: a screen set too low
Start with the screen, because it decides where your eyes go, and your head follows your eyes. If the screen sits below your natural eye line, you drop your gaze to read it — and if the drop is more than a gentle one, your head tips down with it and stays there. That forward-and-down head is the posture OSHA’s neutral one is specifically not.
OSHA gives the target as numbers you can actually check: the “top of the monitor should be at or slightly below eye level,” and the center of the screen “15 to 20 degrees below horizontal eye level.”Put plainly, the top of the glass wants to sit about level with your eyes when you sit up and look straight ahead, so your gaze falls a little rather than dropping to your chin. A lot of setups miss this in the same way: a monitor on its factory stand, an all-in-one at a fixed height, a screen left low because nobody thought to raise it. One caution, because it cuts both ways: too high is also a problem. A screen propped up too far tips your head backto read the top of it, which trades a forward-neck ache for a different one and opens your eyes wider into the bargain. That is why OSHA gives a band rather than “higher is better.” Aim for the top of the glass at about your eye line, and let the rest fall into place.
The laptop problem, which is the screen problem made worse
A laptop used flat on a desk is the screen-too-low problem in its most extreme form, and it is worth calling out on its own because so many people now work this way all day. A laptop’s screen is bolted a hand’s width above its keyboard. Set the keyboard where your hands want it — low, near elbow height — and the screen is now far below your eyes, so you spend the entire day looking down at it. There is no posture that fixes a laptop flat on a desk, because the machine itself puts the screen and the keys in two places your body wants them not to be.
The fix is to stop asking one device to be in two places. Lift the screen up to eye level on a laptop stand, and add a separate keyboard and mouse down at desk level. Now your eyes come up to a screen near eye height while your hands stay low and relaxed — the two things a bare laptop makes you choose between. This is the single highest-value change most laptop-only workers can make for their neck, and it is the reason the laptop stand exists at all. Whether it relieves any pain you already have is not something we can promise; what we can say is that it removes the geometry that forces the head down.
The phone in your lap
Here is the cause people forget because it happens off the clock. You set your desk up carefully, raise the monitor, sit well — and then spend every break with your head dropped over a phone held down in your lap. That is the same forward-and-down head position again, and now the neck is doing it during the hours it was supposed to be recovering.
We are going to stay qualitative here on purpose. There is a lot of confident writing about “text neck” and the exact load a tilted head puts on the spine, and we have not measured any of it, so we are not going to hand you a number we cannot stand behind. The mechanism is the honest part and it is the same one this whole page turns on: a head held forward and down is a head your neck is holding rather than balancing. The practical move is boring — bring the phone up toward eye level rather than bending down to it, the way a stand brings a laptop up — and to treat your breaks as breaks, which is the subject of the last section here.
The chair, the shoulders, and the screen off to one side
The neck does not sit on top of nothing; it sits on top of the shoulders and the back, and both feed into it. OSHA’s neutral posture wants the “shoulders… relaxed” with the “upper arms hang[ing] normally at the side of the body.”A shoulder hitched up toward the ear — usually because a desk is too tall or armrests are parked too high, forcing you to lift to reach — drags on the neck from below. And a lower back that has collapsed into a slump drops the whole chest, which pushes the head forward to compensate, so the neck pays for a problem that started at the pelvis. The other side offender is a screen placed off to one side — a second monitor you actually work on parked to your right, a laptop set at an angle, a document holder across the desk. Anything you look at that is not roughly in front of you is something you turn your head toward, and a head held turned is as far from “forward facing” as a head held down. The cause no adjustment fixes: never moving
You can set every one of the above perfectly and still finish the day with a stiff neck, because the last cause is not a bad position — it is holding any position, even a good one, for hours without a break. A neck held motionless in textbook-neutral alignment for a straight afternoon is still a neck that has not moved. Stillness is its own problem, and no screen height solves it.
The fix is the least glamorous thing on the page and the one we would put first if we could only give you one: change position, often. Look up from the screen, roll the shoulders, stand and walk to refill a glass of water. The best posture really is the next one, and the neutral geometry described above is a good place to keep returning to precisely because you keep leaving it.
